CyberKnife Treatment Delivery Systems equipped with the Xchange Robotic Collimator Changer (all CyberKnife systems running software version 11.X and higher that utilize the Exchange Table) are affected. They were recalled because in certain cases a collimator may not fully dock, triggering a system interlock and, if personnel enter the treatment room and initiate motion with the in‑room control pendant, the collimator may release and fall from the housing. Owners should check whether their system matches the identified software/version and configuration.
