CanadaAffected devices are Medtronic Octopus Nuvo (model TSMICS1; serials 0231547054, 0232670901, 0232784925, 0231912903, 0231758880, 0231792414, 0231758898), Octopus Evo AS (model TS2500; serial 0232709168), and Octopus 4 (model 29400; serials 0231478260, 0231468923). They were recalled because the canister tubing set was misassembled with the short tube attached to the bottom and the long tube attached to the top (reverse order). The hazard
On 7-January-2026, Medtronic identified that the canister tubing set had been misassembled while performing final inspection. The short tube was attached to the bottom of the canister, and the long tube was attached to the top, which is in reverse order than the design specification.
