How to make a reward chart

  1. Write the person’s name and one to three clear goals.
  2. Choose stars, hearts or smiles, and add the reward if there is one.
  3. Colour in a mark each time a goal is met, and start another week when the chart is full.

What’s on the page

Each goal gets a week of large stars, hearts, smiles or boxes to colour in, with a name line at the top and a box for the reward you agree on. Use the whole week or weekdays only.

Options

  • Stars, hearts, smiles, circles or boxes
  • Whole week or weekdays
  • Name line and reward box
  • Four designs, ten colours or your own, four fonts and ruled, dotted or blank lines
